The Oracle HTTP Server instance is down- WEBSERVER (OHS) 12.1.0.1.0 service is up on host but target shows down in EM12c (Agent & OMS version 12.1.0.4.0)
OHS Target shows down in EM12c console whereas the service is up on the
node.

OHS Target shows down in EM12c console whereas the service is up on the
node as shown below.
cd /sid/admin/sid_host/bin
$ ./opmnctl status -l
Processes in Instance: sid_hostname
ias-component status

ohs1 alive
cd /sid/fmw/product/111/ohs_111/opmn/bin
ias-component status
------------- ------
ohs1 alive
$./opmnctl status
Processes in Instance: sid_host
This may be a bug & possible issue may be with the JMX connections as observed from error messages like below in the logs
Caused by: java.io.IOException: Failed attempting connection on serviceURL
service:jmx:t3://host.domain.com:port/jndi/weblogic.management.mbeanservers.domainruntime
